Motorcyclist Injured Sunday Night in Highway 76 crash in Oceanside

JUNE 29, 2009 - At least two people were airlifted from the scene of a motorcycle crash, authorities said. The accident occurred just before 7 p.m. in the eastbound lanes of Highway 76 near Interstate 5, according to the Oceanside Fire Department. Two helicopters were sent to the scene of the crash and transported the injured victims. No further details were available. The California Highway Patrol is investigating the accident.

Bicyclist Riding Without Helmet Seriously Injured Sunday in San Diego

JUNE 29, 2009 - A 32-year-old man was seriously injured after crashing his bicycle into a parked car in the Stockton neighborhood, authorities said. The victim was not wearing a helmet, according to San Diego police. The man was riding at a high rate of speed on J Street near 33rd Street about 2:20 p.m. He was unable to complete a turn in the road and crashed into the parked vehicle, authorities said. Police are investigating the crash.

Man Found Dead on Trolley Tracks Sunday Night in San Diego

JUNE 29, 2009 - A man who appeared to be homeless was apparently struck and killed by a trolley near the Morena station, authorities said. The man’s decapitated body was spotted near the tracks by a trolley conductor about 8:40 p.m., according to the San Diego Police Department. Investigators believe a trolley headed west from the Morena station to the Old Towne station about 10 minutes earlier may have hit the man. Investigators are also looking into the possibility that the man may have jumped between cars and fallen underneath. A bar at the front of the trolley, which detects when something has been hit, had not been tripped, authorities said. The San Diego County medical’s examiners office said the man appeared to be between the ages of 35 and 45. He appeared to be disheveled, leading investigators to believe he was homeless.

Bicyclist Injured in Collision with Car Sunday in Ramona

JUNE 28, 2009 - A bicyclist was seriously injured after colliding with a vehicle, authorities said. The victim was struck about 8:25 a.m. on Ramona Oaks Road near Watt Road, according to the California Highway Patrol. The area is near the Equestrian Center in San Diego Country Estates. The victim was airlifted to Palomar Hospital in Escondido, according to authorities. The extent of the rider’s injuries are unknown. The CHP is investigating the collision.

Teenage Girl Seriously Injured Saturday in Two-Car Crash in Oceanside

JUNE 28, 2009 - A 14-year-old girl was injured when the vehicle she was riding in was hit from behind and overturned, authorities said. The girl was a passenger in a Honda Passport that was rear-ended in heavy traffic by a Camaro, according to an Oceanside police officer. The driver of the Camaro was traveling westbound on State Route 76 at Benet Road about 12:50 p.m. when he failed to slow in time and collided with the Honda, causing it to roll over twice. The victim was ejected from the vehicle, authorities said, and later airlifted to Scripps Memorial Hospital in La Jolla. Three other teenage occupants of the Honda suffered minor injuries and were transported to Tri-City Medical Center. The driver of the Honda and both occupants of the Camaro were uninjured, authorities said. The Oceanside Police Department is investigating the collision.

State of California Fines Encinitas Nursing Home $100,000 for Preventable Patient Death

JUNE 27, 2009 – The San Diego Union Tribune reported this week that the state of California has given an Encinitas nursing home its maximum fine of $100,000 in connection with a patient’s death from a preventable fall, state health regulators have reported. A patient admitted to Aviara Healthcare Center on Regal Road fell two times between May 9 and 10, 2009. He was not injured in the first fall, after which staff put an alarm on his gown. However, staff did not respond when the patient got out of his bed and walked out of his room. The hallway where the man was walking did not have a handrail. The man stumbled and grabbed a piece of equipment in the hallway. The piece of equipment, a device used to lift patients out of bed, fell on the floor and struck the man in the head. He died at a hospital two days later.

Seven employees of Aviara Healtcare stated that the lift was supposed to be stored in another location, and not in the nursing home’s hallway. State investigators found the equipment in the same hallway even after the man’s death. Aviara Healthcare was also fined $16,000 for not monitoring a patient who walked away from the facility three times in one day a month ago.

Passenger Critically Injured When Car Overturned Tuesday Night in Imperial County

JUNE 26, 2009 - A 45-year-old woman from Westchase, Fla. suffered major injuries when the sports utility vehicle she was riding in crashed off Interstate 8, authorities said. The crash occurred about 10 p.m. east of Dunaway Road, when the 2009 Toyota Rav 4 veered off the eastbound lanes, became airborne, struck a perimeter fence and rolled several times, coming to rest on its roof near the frontage road, according to the California Highway Patrol. The unidentified woman, who was not wearing a seat belt, was flown to the University of California San Diego Medical Center, where her condition was unknown Thursday evening. The driver of the SUV, identified as a 41-year-old man from Sierra Vista, Ariz., suffered moderate injuries and was transported to El Centro Regional Medical Center. The CHP is investigating the crash.

Man Critically Injured Wednesday when Motorcycle Crashed on I-5 near Camp Pendleton

JUNE 25, 2009 - An Anaheim man was airlifted to an Orange County hospital after he crashed his motorcycle near the Border Patrol checkpoint, authorities said. The 29-year-old motorcyclist was riding northbound on Interstate 5 at speeds of up to 90 mph when he crashed about 3:30 p.m. while trying to split between a truck and a sports utility vehicle, according to the California Highway Patrol. The man was flown to Mission Regional Hospital in Mission Viejo where he underwent surgery for unknown injuries. The CHP is investigating the crash, but said it appeared the motorcyclist was at fault.

Motorcyclist Killed Wednesday after Rear-Ending Car in El Cajon

JUNE 25, 2009 - A 37-year-old man died after colliding with a car that was turning into a driveway, authorities said. Lance Michael Hagerty of El Cajon was riding a newer-model sport bike on West Bradley Avenue near Wing Avenue about 5 p.m. when he failed to avoid a pickup truck that had slowed to turn into a driveway, according to the San Diego County medical examiner’s office. Hagerty, who was wearing a helmet, died at a local hospital. El Cajon police are investigating the accident.

Sailor Killed in Solo-Vehicle Crash Tuesday Night in Chula Vista

JUNE 24, 2009 - A 24-year-old Coronado man died when his car went off the road and struck a tree, authorities said. The unidentified man was driving east on Otay Lakes Road near Wueste Road about 11:15 p.m., when he failed to negotiate a curve in his 2001 Nissan Maxima, according to the San Diego County medical examiner’s office. The vehicle left the roadway, went down an embankment and wrapped around a tree, authorities said. The man, who was wearing a seat belt, was declared dead at the scene. Officials said the sailor was married and stationed aboard the aircraft carrier Nimitz.
